It is important for anyone over the age of 18 to think about healthcare wishes should a serious illness or injury occur. To help ensure that your wishes regarding life sustaining treatment are followed, Ohio Hills Health Centers provide 2 resources:
1. Advance Directive Packet – This packet is meant to educate you about Ohio’s Living Will, Health Care Power of Attorney, Anatomical Gifts, and Do Not Resuscitate laws
2. State of Ohio Advance Directive Forms - 340B
The Ohio Hills Health Centers 340B Drug Pricing program is a federal program that provides discounts on a select list of drugs when purchased at participating pharmacies. It is available to all patients, regardless of their insurance status or ability to pay. - Referral Processing and Tracking
When an OHHC patient needs to see a specialist, our Referral staff works in a timely manner to ensure the appropriate medical information is sent to the specialist’s office. The Referral staff will ensure that patient’s appointments were completed as part of our tracking processes. A prior authorization is often required by insurance companies for medication, specialty appointments, and procedures before approving payment. To minimize delay in care, OHHC assists in obtaining prior authorizations for our patients. - Interpreter Services
Ohio Hills Health Centers (OHHC) offers sign language interpretation and language translation services for patients who may have communication difficulties to ensure effective communication. - Community Resource Guide
