Ohio Hills Health Services Offers Medicare Annual Wellness Visits

Ohio Hills Health Services is offering Medicare Annual Wellness Visits. This is a free service to all Medicare recipients and is focused on prevention and how to keep Medicare patients healthy. During this visit a review is conducted of an individual’s medical and family history, risk for certain diseases, current state of health and their plans for staying healthy.

The Medicare Annual Wellness Visit is designed to help prevent disease and disability based on a patient’s current health and risk factors. This visit includes, height, weight, blood pressure measurements, a simple vision test, a review of the risk for depression, a home safety assessment, and an opportunity to discuss Advance Directives. A written plan will be provided to the patient recommending the screenings, shots, and other preventive services appropriate for them.

“The Medicare Annual Wellness Visit is a tremendous opportunity for area residents to take a proactive approach to their health,” said Becky Hurley, LPN, Quality Improvement/ Patient Care Coordinator. “Too many people only see their healthcare provider when they are sick. The Wellness Visit helps patients to identify risk factors and provides them the tools to maximize their health.”

All four Ohio Hills Health Services sites- Barnesville Family Health Center, Freeport Family Health Center, Monroe Family Health Center and Quaker City Family Health Center are participating in the Medicare Annual Wellness Visits. This is a comprehensive visit and may take up to one hour. Patients are asked to bring a list of all their health care providers, all their medication bottles and a list of all of the over the counter vitamins and herbs they are currently using.
